This contract, when completed, signed and returned, commits MCO Productions to providing disc jockey services on ___________ from ____ until ____ at ____________________. These services include a professional sound system of suitable power for an expected crowd of ____ persons, a sizable music library, and a master of ceremonies. The customer agrees to maintain adequate security to insure the safety of the equipment, vehicles, and personnel of MCO Productions. MCO Productions will arrive at ______ for set up. Electrical service rated at 120 volts AC, 60 cycles, 20 amps will be required for entertainment. The customer is required to pay a deposit of $200 to put this contract in force.
